JOB DESCRIPTIONS:
We are urgently looking for the qualified Social Worker to start working ASAP.
REQUIRED QUALIFICATIONS:
The applicant wishing to apply for this job must meet the following qualifications;
- SACSSP Registration
- Professional Indemnity Insurance
- Training and experience in Palliative Care.
REQUIRED SKILLS AND COMPETENCIES:
The applicant must have the following skills and competences
- Proven ability in Hospital Social Work and Case Management
- Strong report writing and time management skills
- Comfortable working flexible hours within normal business times
- Willingness to travel for outpatient consultations
- Ability to collaborate effectively with an interdisciplinary care team.
DUTIES AND RESPONSIBILITIES:
The successful candidate will have to implement the following duties but not limited to:
- To assess Care needs
- To assist families to implement recommended care to ensure safe discharges are in place.
- To connect patients to relevant resources such as care agencies, frail cares, day clinics, palliative care resources etc.
- To make referrals to appropriate service providers such as psychologists, Department of Social Development etc.
- To provide short term counseling
- To provide mediation to patients and families
- To provide practical assistance to patients to reduce stressors in order to participate in their rehabilitation.
